Assessment of renal function is an important part of evaluating patients with kidney disease. The measure used to assess renal function is the glomerular filtration rateץ
The GFR is a measure of the functioning of the renal mass and participates in blood filtration processes. The standard GFR is about 120-130 ml per minute corrected for body surface area. Normally, the GFR decreases with age at a rate of 0.75-1 ml per minute per year, after the age 40.
Decrease in GFR is sometimes an early marker of kidney disease. At the same time, fluid and mineral balance in the body and the amount of urine may remain normal until the advanced stages of the disease. Serial GFR monitoring is used to assess the severity and progression of the disease. Decrease in GFR reflects progression or deterioration of the underlying disease, and may also indicate an another acute problem that is developing. An increase in GFR could indicate an improvement in renal function.
GFR assessment is also important in determining appropriate doses of urinary excretion drugs. As the GFR goes down, some urinary medicine excretion may decrease, with an increase of it's presence in the blood levels and a risk of toxicity. To prevent this, it is necessary to reduce the dose or avoid the use of these drugs, in accordance with GFR
